Test Fees and Cancellation Policy
The standard PE Squared LLC d/b/a Steven Gamvroulas Polygraph Examination (“Steven
Gamvroulas Polygraph Examination”) polygraph test fee includes a $100.00 non-refundable deposit per test, per person, and is required to schedule an appointment for a polygraph test. Visa, MasterCard, American Express, Discover, and Bank Debit Cards are accepted payment methods.
The remaining balance of the agreed-upon test fee is due in cash at the time of the test. You can also contact me directly to pay by credit or debit card before the exam. Please see the fee schedule at https://sgpolygraph.com/test-fees
Cancellations received less than 48 hours before the scheduled polygraph appointment date and time, no-shows, or missed appointments for any reason incur the total $100.00 non-refundable deposit fee.
Additionally, arriving more than 15 minutes late to the scheduled polygraph test appointment time without notification will constitute a no-show, and the fee is non-refundable. There is no additional deposit to reschedule an appointment canceled before 48 hours if scheduled within seven calendar days.
No additional services or consultations are included in the polygraph test fee once the following polygraph test is concluded and the results are provided for relationship polygraph testing.
Terms and Conditions
The client and the examinee voluntarily, without threats, promises of immunity, or reward, and without duress, coercion, force, or threat of adverse action in the event of a failed test, agree to submit to a polygraph examination to be administered to me by any examiner of PE Squared LLC d/b/a Steven Gamvroulas Polygraph Examination (“Steven Gamvroulas Polygraph Examination”). Both examinee and client understand the examinee is not required to take the polygraph examination, and the examinee has the right to consult their attorney or any other party they wish to before taking the polygraph examination or signing any forms. The examinee and client at this moment voluntarily request and authorize any polygraph examiner conducting the polygraph examination, Steven Gamvroulas Polygraph Examination, and any of its officers consent to audio and video recording devices during the pre-test interview, examination, and post-test process. The examiner has the examinee’s and client’s permission and consents to be asked questions during the pre-examination interview, during the testing portion of the examination, and any post-examination interview as deemed necessary to verify the examinee’s past work record, criminal record, financial condition, military service, general honesty, medical background and any other information that is pertinent to the polygraph examination. The examinee voluntarily authorizes the examiner to administer a polygraph examination and grants permission to attach the blood pressure cuff, electrodermal activity sensors, pneumographs, and other sensors needed to conduct the polygraph examination test to or on the examinee’s body. The examinee now expressly waives all rights of privacy that the examinee has or may have regarding taking the examination and making known the results and the examiner’s opinion. Examinee authorizes Steven Gamvroulas Polygraph Examination, its officers, employees, and any other examiners to release the exam recordings and to disclose both orally and in writing the results to the client or any interested persons or parties. All videos, charts, and other materials generated by the examination are the sole property of PE Squared LLC DBA Steven Gamvroulas Polygraph Examination. The polygraph examination test results will be reported from the following: No Significant Reactions/No Deception Indicated, Significant Reactions/Deception Indicated, Inconclusive/No Opinion Formulated (Inadequate physiological reactions by the examinee), or Purposeful Non-Cooperation (The examinee did not cooperate during the examination process).The examinee agrees to behave appropriately from the arrival until departure from 2319 South Foothill Dr. Any threatening or aggressive behaviors, intimidation, criminal conduct of any kind, harassment, or any behaviors deemed inappropriate, unprofessional, or unlawful in a business setting, will be grounds for immediate ejection from 2319 S Foothill Dr. A criminal complaint and civil action may be pursued against any individual who engages in making defamatory statements, defamation of character, or false statements about any examiner and or any employee of PE Squared LLC DBA Steven Gamvroulas Polygraph Examination, whether verbally, in writing and or on-line. The examinee agrees to cooperate fully with the examiner during the polygraph examination process. Intentional manipulations by the examinee during the testing process may be a reason for immediate termination. The examination process begins at the appointment start time in our offices and ends with the presentation of the polygraph test results and receipt of the final report by email. For relationship polygraph testing, no additional phone consultations or other services are included in the examination fee following the examination process in our office. Knowing this fact, the client and examinee acknowledge that any communication received by Steven Gamvroulas Polygraph Examination, or any of its examiners, officers, and or employees following the examination process in our office from either the client, the examinee, or any third party inquiring on behalf of either the client or examinee may be automatically billed to the client’s credit card on file used to pay for or reserve the polygraph examination or invoiced at an hourly rate of $400.00 in minimum 15-minute increments. Any other consultations, subpoena responses, compliance with subpoenas, interviews, court appearances, and other services will be billed at an hourly rate of $400. The total examination cost is non-refundable for cancellations received less than 48 hours before the scheduled test time, missed appointments, or if the examinee and or client arrive after their scheduled appointment time without appropriate notification. The total examination cost is non-refundable regardless of the early termination of the examination process by either the client or examinee. In consideration of Steven Gamvroulas Polygraph Examination and any of its examiners, officers, and or employees, the examinee and client do at this moment RELEASE, WAIVE, DISCHARGE, AGREE NOT TO SUE and FOREVER to hold those above free from all harm, liability, or damage to the examinee and or client as a consequence of the polygraph examination, release of the recordings, release of the results, release of the written report and or examiner’s opinion, Steven Gamvroulas Polygraph Examination, together with its examiners, officers and or employees. Examinee and client do at this moment RELEASE, WAIVE, and FOREVER DISCHARGE Steven Gamvroulas Polygraph Examination and any of its examiners, officers, and or employees from any suits, actions, causes of action at law, claims, demands, the liability of any kind or description which examinee or client have now or may have to result directly or indirectly from the polygraph examination, operation of all recording devices, releasing of the recordings, the oral and written opinions rendered, releasing of the written report and all future actions taken by the examinee, client or anyone else because of the polygraph examination. Examinee and client further, at this moment, AGREE TO INDEMNIFY AND HOLD HARMLESS Steven Gamvroulas Polygraph Examination and any of its examiners, officers, and or employees from any loss, liability, lawsuits, damages, or costs, including court costs, and attorney’s fees, that they may incur due to the polygraph examination, the release of the recordings, release of the results, the release of the written report and or examiner’s opinion and WHETHER CAUSED BY NEGLIGENCE OF Steven Gamvroulas Polygraph Examination and any of its Examiners, officers and or employees
